We have a slate fireplace which needs to be affixed again to the wall and the chimney hearth tiles repaired.
Does anyone have any recommendations for a Chimney sweep/repair person that they have used please?

H Firkins & Sons are good and reliable (now onto their second generation). I think more on sweeping than repairs, but they may be able to recommend someone for that.
I invited them to sweep my chimney when I moved in - they found (a) the chimney had been blocked off at the top and (b) there were no bricks behind the cast iron hearth set into the wooden mantlepiece/surround. Lucky I didn't light a fire first!
Anyway they got someone round within hours to fix those problems, then the swept the chimney, all at a reasonable price, plus a few return visits. All well and safe now, I don't use the fire anymore because of pollution concerns (I'm upstairs so never a good draw), but that's another discussion.
I understand this was more of a building job, and you need tiles repaired, but it may be worth asking.
